Advancing Health Equity through Community Engagement

At SBH Health System, we believe that our community is not just a place, but also a vibrant tapestry of diverse cultures and experiences. We’re committed to serving Bronx residents with events, programs and resources that truly reflect their values and needs.

A culturally informed approach to advance health equity honors community members’ knowledge and abilities. Such an approach can increase the sustainability of our efforts. Taking a community-driven approach to health equity means recognizing and respecting the knowledge and skills of local people. This way of working together can make our efforts stronger and last longer. We create solutions that truly meet our community’s needs and help everyone live healthier lives.

As one of the largest organizations in the community, we partner with other local institutions to share resources and create programming that enhances the daily lives of Bronx residents. Additionally, every three years, we participate in a community needs assessment based on survey data from local citizens. We then create initiatives that directly address survey feedback, based on the demographics and nuances that define our neighborhoods.

We always welcome suggestions for additional programs and resources. For more information, please reach out to our Community Engagement Team at 718-960-3929 or email [email protected]. We look forward to working with you to nurture a vibrant and inclusive community where everyone’s health and well-being thrive.

I wanted to say thank you so much for being a wonderful team maker, mentor and support for networking. I have enjoyed the experience in SBH Hospital. I was so fortunate to have you as my Preceptor during my internship.

SBH Health System Wellness Alliance

At SBH Health System, we believe in keeping our community safe and healthy. In response to the community’s concerns and needs we’ve teamed up with local programs to create a Hospital Based Violence Prevention Program. Together, this initiative, we’re working to reduce violence, support those affected, and make our neighborhood a better place for everyone.

Hospital Based Violence Intervention Program (HVIP)

SBH in partnership with Bronx Rises Against Gun Violence (B.R.A.G)/Good Shepard Services, SBH Emergency Medicine and Community Affairs Departments operate a Hospital Based Violence Prevention program. The initiative with B.R.A.G. is a direct response to the community needs assessment and the feedback of concerned citizens.

Health Fairs and Screenings

SBH Health Systems’ goal is to help our community not only get well but stay well. We bring health education and preventive health care directly to our neighborhoods with on-site hearing, dental, blood pressure, mobile mammography and vision screenings. All community members are invited and encouraged to participate in our free health fairs and screenings, no matter the location.

Community Service Plan

SBH Health System commits to further priorities initiatives with coordinated evidence-based programs consistent with the New York State Department of Health Prevention Agenda.

Every three years, SBH conducts a community health needs and assets assessment. In response, SBH prepares a community service plan to address healthcare community needs.

The projects focus on advancing health equity by addressing prevention of chronic disease, promoting access to quality health care services and designating measures to eliminate health care disparities. The selection of priority projects results from a review of demographic and health data and subsequent discussions with community partners.

The plan monitors to assess the progress of the initiatives and makes necessary adjustments to improve health outcomes for Bronx residents.
