Our dedicated team offers referring providers a concierge, closed-loop referral service with access to our wide array of clinical specialists to ensure that your patients have a seamless and high quality care experience. Our referral team works directly with patients to schedule appointments at convenient times and ensure they have what they need for their visit. We also offer referring providers secure access to our EpicCare Link portal where they can rapidly access their patients’ information, including hospitalizations, specialist referrals, and real-time clinical data.
EpicCare Link is a secure web portal for SBH Health System’s new Epic electronic medical record that enables referring providers and staff to easily refer patients to SBH Health System. You can conveniently access patient information for your referred and admitted patients.
